Instructions for the Application Form:
Name: Your name as you wish it to appear on your certificate and membership
card.
Main Job, AOC, MOS: Your primary job title.
Highest Level of Education Obtained: Your highest degree or level of training.
Preferred College of Assignment: Usually the same as your field, but in
some cases education and experience may justify a different assignment.
Contact Phone Number: The phone number to reach you as a member or the contact
representative for a unit or office membership.
Contact Email: The email number to reach you as a member or contact representative
for a unit or office membership.
Contact Mailing Address: The address to send your member or unit/office
certificate of membership and membership card.
Do You Wish To Participate in Academy Task Forces? Academy Task Forces
study specific problems related to the different areas of Disaster Incident Management and Mass Casualty Care. Results will
ultimately be published in the Academys journal.
Membership Application Fee: yearly and self explanatory for individuals.
Office or Unit Membership: allows a group from or of an office or unit to
pay the same individual fee for the whole outfit, designating one individual as the contact representative for the group.
